Generic vs. Brand
"Generic" gets treated like a downgrade, but it's actually a legal specification with strict requirements. Here's what FDA's approval standards really require — and where the real (usually minor) differences can show up.
FDA's generic approval standard covers four areas: pharmaceutical equivalence (same active ingredient, dosage form, strength, route, and indications), bioequivalence (human studies showing it performs the same in the body), labeling (the same label as the brand's, with limited exceptions), and manufacturing (the same quality requirements for identity, strength, purity, and consistency).
What to do instead: Stop treating “generic” as a downgrade tier and treat it as a specification. Your pharmacist can show you that the active ingredient and strength on both Drug Facts panels are identical.

FDA acknowledges small, allowable differences exist between products from different companies, including certain inactive ingredients, and that in most cases they don't matter clinically. But FDA also says the honest second half out loud: in a minority of patients, inactive ingredients may affect tolerability or absorption — usually minor and uncommon, but “worth clinical attention if patients report concerns.”
What to do instead: Don't abandon generics as a category — take the bottle to your pharmacist, who can compare inactive ingredient lists between versions and often order a different manufacturer's product for you.

FDA spells out the economics: generic manufacturers can sell for lower prices because they don't repeat costly clinical trials, generally don't pay for advertising and marketing, and multiple generic companies competing for the same product pushes prices down. The average cost of a generic is 80–85% less than its brand-name counterpart — while manufacturing sites must pass the same quality standards as the brand's.
What to do instead: Read the price gap as a marketing-and-patent story, not a quality story. Ask your pharmacist which OTC or generic version delivers the same active ingredient for less.

FDA defines an authorized generic as an approved brand-name drug marketed without the brand name on its label. Other than the missing brand name, FDA says it is “the exact same drug product as the branded product,” marketed under the brand's own application rather than a generic application — either by the brand company itself or with its permission.
What to do instead: If you're brand-loyal but price-sensitive, ask your pharmacist whether an authorized generic exists for what you're buying — it's the same product from the same application, minus the logo.

FDA defines narrow therapeutic index (NTI) drugs as ones where small dose differences may lead to serious therapeutic failure or life-threatening reactions. The examples FDA lists — warfarin, phenytoin, levothyroxine, lithium, digoxin, and others — are all prescription medicines, not OTC products, and face a stricter generic-approval path than standard drugs.
What to do instead: For OTC purchases, choose on active ingredient and strength and buy the cheaper version with confidence. If you also take a prescription NTI medicine, ask your pharmacist to confirm the manufacturer before assuming anything changed.
